Joyce Ann Havlicek

of Phoenix, AZ, born May 26, 1962 in Chicago, Illinois, passed away at the age of 54 on July 21, 2016. She is survived by mother Rose Marie Havlicek (preceded in death father Charles James Havlicek), sister Charlette (husband Val) LeChuga, brother Richard (wife Brenda) Havlicek, Sr, nephews Richard Havlicek, Jr. & James Havlicek, and great neice Kiara Havlicek and great nephews Richard Havlicek, III and Xavier Havlicek. A Memorial Service will be held at Whitney & Murphy Funeral Home, 4800 E. Indian School Road, Phoenix on Saturday, July 23, 2016 at 11:00am.
