96, of Phoenix passed away on July 27, 2016, surrounded by
family. He is survived by his wife Eileen of 73 years, two sons, Dr. John M. Kozak (Cathy), Dr. Paul A. Kozak (Jeanna), granddaughters Chelsea and Chase, grandsons Jacob and Nicholas, step-grandchildren, Zachary, Grace and Caroline.
Frank honorably served our Country in the Army during WWII for 5 years. He owned and operated his own accounting and auditing business in Boston, MA. Later, he retired and
moved to Phoenix in 1996. Frank was a proud member and moderator of the First Congregational Church of Chelsea, MA. Arrangements entrusted to Whitney & Murphy Funeral Home.
