36, of Phoenix, passed away on August 23, 2016. Jack is survived by his parents Michael and Katherine, sister Suzanne, brothers Michael Jr., Paul and Mark. A Memorial Mass will be celebrated at 10:30 A.M. on Wednesday, August 31, 2016 at St. Thomas the Apostle Catholic Church, 2312 E. Campbell Avenue in Phoenix.
